WORK AUTHORIZATION AND SERVICE DISCLOSURE
Cart and Camper Solutions, LLC
(including its divisions PA Golf Cart World and NomadicRVTech)

Authorization
The  undersigned Customer (“Customer”) authorizes Cart and Camper Solutions,  LLC (“Service Provider”) and its authorized personnel to perform the  services listed in the Work Order or Estimate, including the use of  necessary parts and materials. Customer further authorizes operation of  their RV, RV system, or Golf Cart for testing, inspection, and delivery  at Customer’s sole risk.

Warranty

Estimates are valid for 10 calendar days.
Labor performed is warrantied for 30 days from completion.
Diagnostic findings are warrantied for 5 days.
Parts  warranties** vary by manufacturer and must be processed directly  through the manufacturer. Service Provider is not responsible for  manufacturer warranty determinations, delays, or denials.
**Important:  While a part may be covered under a manufacturer’s warranty, any  related service call, travel, or labor charges necessary to replace or  install the part are not covered once the Service Provider’s labor  warranty period has expired.
 

Liability / Waiver of Insurance Coverage
Customer acknowledges that Service Provider does not maintain Garage Keeper’s Insurance.
Customer  expressly agrees to waive, release, and hold harmless Service Provider,  its officers, employees, and contractors from any and all liability for  loss, damage, theft, fire, vandalism, accident, weather-related events,  natural disasters, or any other unforeseen occurrence while the RV, RV  system, or Golf Cart is in the possession, custody, or control of  Service Provider.
Customer agrees that personal property left in or  on the unit is not covered by any insurance or liability of the Service  Provider. Customer is advised to remove all valuables prior to service.
Service Provider’s liability, if any, shall be limited solely to cases of gross negligence or willful misconduct.
 

Payment Terms
Full payment is due upon completion of services.
Accepted payment methods: Cash, Check, Visa, and MasterCard.
A 3% processing fee applies to all credit and debit card payments.
Returned  checks will incur a $50 NSF fee and must be repaid in cash within 7  calendar days. Failure to make timely payment may result in legal  action.
 

Storage & Mechanic’s Lien
Units left more than 5 days after notice of completion may be subject to storage charges of $25 per day.
Customer  grants Service Provider an express mechanic’s lien on the RV or Golf  Cart to secure payment for all authorized services, labor, parts, and  materials under the laws of the Commonwealth of Pennsylvania.
 

Jurisdiction
This  Agreement shall be governed by and construed in accordance with the  laws of the Commonwealth of Pennsylvania, with venue for any dispute  exclusively in the courts of Beaver County, PA.

Acknowledgment
By  clicking on the agree box found at the bottom of the Service Request Form under this duplicate Disclosure, Customer acknowledges they have read,  understood, and agree to all terms set forth in this Work Authorization  and Service Disclosure.
